We need the following items for your file:

1. Please send us a copy of your certificate of insurance for general liability in the amount of $3 million.
2. For Burton 1 & 2 please list the following as additional insured:
   • Burton 1 & 2, LLC; Eakin Partners, LLC; and Eakin Properties, LLC.
3. For Burton 6 please list the following as additional insured:
   • Burton 6, LLC; Eakin Partners, LLC; and Eakin Properties, LLC.
